Night Shift Licensed Practical Nurse (LPN) or Registered Nurse (RN)

As an important member of our interdisciplinary team, the LPN or RN delivers quality nursing care to patients through interpersonal contact and provides care and services to allow patients to attain or maintain the highest practicable physical, mental, and psychosocial well-being in accordance with all applicable laws, regulations, and The Peaks standards.

Your role will involve monitoring and reporting changes in the patient’s condition on a daily basis, you must be able to assign duties, instruct, and provide clinical supervision to CNAs.

**Job Summary:**
Grace Manor Care Center is seeking a full-time Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our NIGHT SHIFT team. As an LPN or RN at Grace Manor Care Center, you will have the opportunity to provide compassionate care to senior residents within our community, becoming an integral part of their extended family.


**Responsibilities:**
- Monitor patients' conditions and assess their needs for optimal care and advice.
- Observing and interpreting patients' symptoms, communicating effectively with physicians.
- Collaborate with healthcare team to develop individualized care plans.
- Conduct routine procedures, administer medications, and maintain accurate patient records.
- Ensure facility cleanliness and safety standards are met.
- Provide immediate medical care during emergencies.
- Supervise nursing assistants and maintain a supportive environment for patients and families.
- Continuously expand knowledge through educational opportunities.

**Qualifications:**
- Active State Issued LPN or RN License.
- Nursing Degree from an accredited college or University.
- High School Diploma or Equivalent.
